Health Service Fees
Fee for Service effective February 11, 2013
We accept cash, check (except for birth and death certificates), or credit card (3% convenience charge applies).
Office Visits (no one will be refused services for inability to pay for office visits or childhood immunizations, with the exception of influenza shots)
Childhood Immunizations Private insurance billing, Medicaid billing or if self pay, ask when you call for pricing.
DtaP, Pediarix (DtaP, Hep B, and IPV), IPV (Polio), Hepatitis A, Hepatitis B, HIB, HPV (Gardasil)(VFC eligible ONLY), MCV4, (Menactra), MMR, PCV 13 (Prevnar), Rotavirus Rotateq), TD, TDAP, Varicella.
Immunizations for Adults Private insurance, or self pay, ask when you make an appointment if you need to know cost.
Hepatitis A, Hepatitis B, TD (Tetanus), TDAP, TWINRIX (Combo Hep A and Hep B), PNEUMOCOCAL, Shingles, Influenza (available for all ages)

The Callaway County Health Department can print birth and death certificates upon request for a service fee.
