Individuals may apply for certified birth and death certificates at the Callaway County Health Department. Same-day services are available. It typically takes about 15 minutes to complete the request in person.

Missouri Birth Certificates
General Guidelines and Cost:
Must be born in the State of Missouri.
Certificates are available dating back to 1920. For prior records, contact Vital Records.
For newborns, please allow 4-6 weeks for processing before applying.
$15.00 per certified copy.
Missouri Death Certificates
General Guidelines and Cost:
Must have died in the State of Missouri.
Certificates are available dating back to 1980. For prior records, contact Vital Records.
$14.00 for the first copy.
$11.00 for each additional copy at the same visit.

Identification Requirements
Primary Identification: Provide ONE (1) of the following:

- Valid state-issued driver’s license
- State-issued ID card with photo and date of birth
- U.S. military ID card that includes a photo
- U.S. passport with photo
- School ID card/document with applicant’s name, photo, and school year
- Work ID card with applicant’s name, photograph, and company name
Alternate Identification: Provide TWO (2) of the following:

- Social Security card
- Medicaid/Medicare card
- Utility bill
- Pay stub
- Rental lease
- W-2 form
- Bank statement
- Car registration or title with current address
Missouri Law
Missouri law only allows a certified copy of a vital record to be issued to a person with a direct and tangible interest in the record. The application for obtaining a copy of a vital record requires applicants to explain their relationship to the person named on the record. Obtaining a record illegally is a Class D felony.
